"Cassius" Pence has declined to appear on Trump's behalf as per instructed, at Ryan's campaign event in Wisconsin, saying that he "cannot defend" Trump's remarks.
This is big. I think it may well be the beginning of a concerted effort by the RNC to "fire" Trump and Run Pence in his place.

They can withdraw all financial, logistic, strategic and media support and instruct their pet donors to do likewise. This would cause Trump's campaign to go down in flames, something his ego could not tolerate.
The only question I have is how the GOP establishment feels about Pence. That fucker's crazy.

What would happen if Trump all of the sudden was no longer able to run for president - say a heart attack and he died? What would happen?

The cost to reprint ballots would be enormous, as many states have opened absentee voting, so the party is stuck with Trump ON THE BALLOT. That said, I'm sure both parties would be amenable to his "resigning" after the election, and having Pence serve if that voting result ever came to fruition.
Trump just shot his foot off with an Uzi, and I don't doubt that Hillary has another four or five of these bombs around. The Reps have already cancelled all "Trump Wins!!!" contracts and associated work, because they now know that reality is about twenty alternate universes away.
I'm beginning to have a lot of questions about the the Republican Party's vetting processes. How the hell could they allow first Palin, then the Tea Party, and now Trump, become their flagbearers in the past decade. The party is so wacked out they need to lose in a landslide, so they can start from scratch and build a new machine.
And J, Pence would slide to the top, while Christie or Cruz would take the VP slot for expediency.
