I don't get why anyone has 'exclusive rights' to manufacture a drug that's been on the market 62 years.

It seems like one looking to the protection of the laws for his patent should have a concomitant obligation to accept limits on his monopoly when the public interest demands it. Medical care is one of those areas where market forces cannot be the only forces affecting the distribution of a necessary product.

I thought that at this point in our history such legal provisions would already be in place.

I don't really practice in intellectual property, but my understanding is that the patent laws grant a legal monopoly to the patent holder for the duration of the patent.
Unfortunately, in our history, no forces are more revered than market forces. If the market will bear $750 per pill, then the market has spoken. The circus surrounding the negotiation of the ACA demonstrated how necessary it was to retain full fledged capitalism as a central feature in order to get it passed.
To expect anything other than the almighty market to determine value is just my Utopian pipe dream.

Maybe 'exclusive rights' are not protected by patent in this case. Maybe they are protected by the FDA, because no one else is certified by them to manufacture the drug.
Or maybe they are protected by contract, the holder having paid all other certified makers not to manufacture. The last is certainly an 'agreement in restraint of trade,' but i know such agreements are permitted sometimes in the pharmaceutical industry.
